OCuSOFT, Inc., respects that privacy is important to you. Our
primary goal in presenting this website is to share helpful information

Notice of Privacy Practices Effective As of March 2, 2007
OCuSOFT, Inc., through optional registration, may gather personally
identifiable information about you such as name, address, telephone
number, e-mail address, date of birth, or other similar information
("Information") when you voluntarily submit it to us
or when you give permission to others to share such Information
with us. The Information we collect may be used to send you health-related
materials, to develop products, services and programs that you
may find useful, or to contact you. From time to time we may also
request that you volunteer additional demographic, or other health
information that we may use to serve you better. The Health Insurance
Portability and Accountability Act (“HIPAA”) protects
your individually identifiable health information (“protected
health information”). HIPAA outlines when OCuSOFT may or
may not use or disclose your protected health information. HIPAA
requires your written authorization for uses or disclosures of
your protected health information not allowed under the law. OCuSOFT
can use and disclose any information which is not individually
identifiable. OCuSOFT agrees to protect your individually identifiable
health information by using and disclosing it only as permitted
in this Policy and as directed by state and federal law.
Any Information you provide to us is held on a secured web server
(a computer that stores web documents, allowing users to access
web pages using web client software) operated by OCuSOFT, Inc.
(or by an agent of OCuSOFT, Inc performing services on our behalf).
We will not transfer, sell or rent Information to anyone, other
than our affiliated companies, and external individuals and organizations
working on our behalf, without your permission.
Through our web sites, OCuSOFT, Inc. also collects non-personally
identifiable information relating to such matters as: the total
number of visits to our web sites, the number of visitors to each
page of our web sites, the Internet Protocol (IP) addresses of
our visitors, and the time spent on our web sites. We use this
information to understand how our visitors use our web sites so
we can improve the customer experience. We may also use your IP
address to help diagnose problems with our servers, and for purposes
of system administration. From time to time we may also share non-personally
identifiable information with others.
This website uses Internet Protocol (IP) Addresses. An IP Address
is a number designated for your computer by your Internet service
provider to provide access to the Internet. Generally, an IP address

we use, may contain information that may be deemed identifiable.

connections, your IP Address would not change and could be associated

How OCuSOFT may use and disclose

Administrative and Business Activities: OCuSOFT is permitted to
use and disclose your protected health information to operate our
business, for example we may:

2) Provide customer service;
3) Operate our website; and
4) Create data summaries, and analyze data using individually
unidentifiable health information which is not protected and other
information that you have provided.
OCuSOFT may also share your protected health information with:

2) Family members, in certain situations where the family member

3) The Department of Health and Human Services may request your
protected health information to investigate complaints and to review
OCuSOFT’s compliance with HIPAA;

information. For example, the Food and Drug Administration may
request information related to the quality, safety or effectiveness
of a OCuSOFT product;
5) A law enforcement agency required by law to receive your protected
health information. For example, a court or administrative agency
may subpoena information for purposes of litigation; and
6) A medical examiner or coroner requiring your protected health
information to perform his duties required by law, or to identify
or determine a cause of death.
In order for OCuSOFT to use your protected health information
for any other use or disclosure than those outlined above, OCuSOFT
must obtain your authorization.

information in connection with the sale, assignment, or other transfer
of the business of this website to which the information relates,
in which case we would require any such buyer to treat your information
in accordance with this Privacy Statement.
OCuSOFT will make every effort to protect your privacy and health
information. We train our employees, restrict access to your protected
health information, and use technology to prevent unauthorized
uses and disclosures.
